Our FAST-Track Application has been updated to include projects up to $100,000. Complete and submit for contract bonds under $100,000.

For contract bonds over $100,000, please complete each of the following forms that make up our Contract Bond Set-Up Kit. All submissions should be to Lanzo, Lutz & Associates Insurance Agency, your Pennsylvania Connection for Contractors Insurance. We will check the application for accuracy and submit it for approval and issue.  Please submit these forms along with the company financial statement and resumes on owners/key personnel (companies 5 years or older are exempt from sending resumes).

PLEASE NOTE: A complete submission includes the forms below, financial statements and resumes. To expedite the approval process, please furnish us with a complete submission.

The Form 10 Application should be used when applying for the following types of bonds:

Individual Public Official
Court, Probate/Fiduciary
License and Permit
Bankruptcy Trustees, Receivers
Lost Securities Bonds
Notary Bonds
Probate Bonds Under $500,000
Public Official Bonds Under $75,000

Complete the front of the application and the relevant section on the reverse side. In some cases, the balance sheet portion will also need to be completed. Be sure to include a proper signature on the front of the application.
